Since its establishment in 2008, Equal Hope has been dedicated to eliminating health inequities throughout Illinois specifically in the Chicagoland area, with a mission to save lives. By conducting research, implementing quality assessments and improvements, advocating for change, providing education, and offering patient navigation services, Equal Hope takes a comprehensive approach to achieving sustainable outcomes and true health equity. Each year, Equal Hope reaches over 60,000 individuals, provides education to 35,000 individuals, and guides 1800 individuals to preventive health screenings and primary care.
